What to Bring
Your first visit is comprehensive. Please allow extra time so Dr. Kelchen can give your case the thorough attention it deserves. Follow-up visits are typically 15–30 minutes.
Office Policies
Appointment Cancellations
We ask for at least 24 hours notice for cancellations. This allows us to offer the appointment to other patients in need.
Late Arrivals
Please arrive 10 minutes early for your first visit to complete paperwork. Late arrivals may need to be rescheduled.
Payment
Payment is due at the time of service. We accept cash, check, and all major credit cards. We also offer flexible payment plans.
Insurance Verification
We will verify your insurance benefits before your first visit. Please call us with your insurance information so we can confirm coverage.
